Before filling out our online registration forms below, please be sure you have the following:

• a stable internet connection;

• a WORKING personal email, preferably Gmail; one per learner; only one parent/guardian per family needs a Gmail account to register during onboarding;

• a hi-res softcopy file of your original PSA-issued BIRTH CERTIFICATE; old copies and baptismal certificates will be accepted TEMPORARILY;

• a hi-res softcopy file of your ID photo, passport size, with plain white or light colored background, and with name tag at the bottom frame;

• softcopy file attachments should be in jpg, png or pdf formats.

PLEASE BE VERY CAREFUL AND THOROUGH IN FILLING OUT THE FORMS. 

Most registration problems are caused by misspellings and wrong data.
The personal data you will input should ALWAYS be based on the OFFICIAL DOCUMENTS you will submit to us.

The prospective ALS LEARNER should be the one to fill out the form.  
If the learner is too young, a parent or guardian may fill out the form but have the learner beside so he/she can learn how to fill out forms.  Document this process because this will come in useful in the ALS learning process.
